This form should be used for all campus events. To request an event you MUST COMPLETE ALL FIELDS on the Event Requisition. Once all fields have been completed, click "Submit" and the form will be automatically submitted. You will receive an email acknowledging your request, but your event HAS NOT BEEN SCHEDULED until you receive a second email specifying approval. Submitting this form does not guarantee the scheduling of your event.
